Quality Service Facilitator

Working Conditions:

  • Flexibility in scheduling: This is a Contract FT position. Schedule will consist of hours throughout all shifts and will include weekend and holiday hours
  • Able to work independently and in a team. Must be self-directed
  • Walking and standing for long periods

The facilitator’s job is to support everyone to do their best thinking and practice to ensure the residents and customers of the Home receive the best care and services possible; improving performance by turning things people do (or don’t do) into learning situations in a planned way under guidance. This is a management role .

Responsibilities and Duties

Within the facilitator’s scope of practice:

  • To assist the Nursing Leadership Team in development, implementation and evaluation of nursing care and services programs
  • To monitor the care and services provided to the residents, through direct supervision and formal and informal audits
  • To promote the Mission, Vision and Values of the Home
  • To promote compliance with the LTC Homes Act, Occupational Health and Safety Act and other legislation governing the organization
  • To promote improved practice through mentorship, coaching and education
  • To facilitate conflict resolution between customers (staff, residents, visitors, etc)
  • To report concerns and issues outside scope of practice to the Director of Care or designate
  • Continuous learning to broaden knowledge and scope of practice related to their peer group
  • To participate in corrective action of employees
  • Involvement in committees, as directed by the Director of Care
  • To promote teamwork among nursing personnel and between departments
  • To assist in complaint and compliance investigations, including but not limited to, providing witness testimony where required
  • This role reports directly to the Director of Care and under direction, to the Assistant Director of Care

Qualifications and Skills

Qualifications

  • Must be a Registered Practical Nurse with previous experience in Long Term Care.

Skills

  • Good understanding of the Long Term Care Act and regulations, policies and procedures of the Home and other legislation that governs the Home.
  • Excellent communications skills, including but not limited to: articulation, ability to adjust their communication to the personality of the listener, excellent English both speech and written, good listening skills, approachability
  • Strong computer literacy and typing skills
  • Demonstrated commitment to quality care and services
  • Demonstrated problem solving and conflict resolution skills
  • Demonstrated understanding of Responsive Behaviour Management
  • Willingness to share knowledge, skills and expertise
  • Positive attitude and acts as a positive role model (leading by example)
